Here's the reply I received from Mark Zitz at Cavtel, who apparently works on Sunday. It's not a particularly attractive offer, so my answer is no, but at least it's polite in tone, and I suppose I should be honored that he'll handle it personally, although I suspect he's aware that his staff would probably screw things up even further.

Dear Mr. Lunsford,
Thank you for your note below. I sincerely apologize for your service interruption and for the information that Virginia relayed to you. Virginia works in my department, and I will follow through on the comments she made to you.
Also, I will follow through on your request, but may I ask you to consider staying with us if I credit your account for one month's line charges? If your line still is not operational, I will have someone contact you at your convenience. Would you kindly let me know your preference for my offer.
If you choose to leave, I will credit your account retroactively to July 22 and process your request immediately.
I will handle this personally.
Thank you,
Mark Zitz
